Instructions for requesting a questionnaire review

The Measurement and Evaluation Unit in the Deanship of Quality and Development is the only reference body entrusted with reviewing and applying electronic questionnaires issued by the university to all target groups (inside or outside the university).
The party responsible for the questionnaire must communicate with the Measurement and Evaluation Unit according to the announced form in order to review the electronic questionnaire required to be implemented.
It is not permissible for any party within the university to design a questionnaire bearing the university’s logo and apply it electronically except through the Measurement and Evaluation Unit.
If the content of the questionnaire requires approvals related to the ethics of scientific research, please contact the graduate study agency and attach proof of this.
The request form for conducting the questionnaire shall be submitted no less than two weeks before the expected date of publication of the questionnaire.
The time plan must be adhered to according to the announced schedule for applying the questionnaires.
An electronic copy in doc format of the questionnaire to be designed is attached.

The following are excluded from this:

Small group questionnaires, called point of service, which result from mini-meetings and brief workshops.
Questionnaires administered by postgraduate students aimed at completing a doctoral or master’s thesis
Questionnaires administered by faculty members for scientific research.
